It's very sweet. And it's not too cheesy, although it might be a little intimidating for a 16 year old boy!! It was a little hard for me to focus on because of all the capitalizations, actually, but for an unedited poem, it's very nice, and I also don't think you really want it edited, because it'll take away from it being something uniquely yours. If you disagree, I'd be happy to help you edit it, but it really does always change the voice. I'm sure he'll appreciate it, although I'll reiterate that, unless he's absolutely wild about you, it could startle him a bit. (:
